Martha's Place & Selah West Reservation Forms

We are excited for your stay at Martha's Place or Selah West, and will strive to provide a fun and relaxing atmosphere!  In order to best serve you, please read, print, and complete all of the following important documents to help us get better acquainted with one another:

• Policies Signature Form
• Intake Form
• "No Violence" Acknowledgement
• House Rules
• Medical Information Form
• Payment Information Form
• Transportation Release

These forms must be completed, signed, and returned to Quiet Waters Outreach at least two weeks prior to your stay (unless other arrangements are approved by QWO's management) in order to confirm your reservation.  

We require all future guests to tour Martha's Place before your stay so that you can familiarize yourself with the surroundings and our staff members.
